Peptide Injection Site Rotation

Why it matters, which zones to use, and how to track it systematically so lipohypertrophy never becomes your problem.

Why Rotation Is Non-Negotiable

Inject into the same site repeatedly and the tissue hardens. Lipohypertrophy — thickened, fibrous fatty tissue — forms under the skin where repeated trauma occurs. Once you have it, absorption from that site becomes unreliable. You can be dosing correctly on paper while half your peptide sits in scar tissue doing nothing.

The fix is not complicated. Rotate across enough zones that each site has several days to recover before it is used again. Track which site you used for each injection so you are not guessing. That is the entire protocol.

The Rotation Zones

For subcutaneous injections, use these zones:

Six zones is the minimum. For twice-daily dosing, use 8–10 so each site has at least 3 full days before you return to it.

The Recovery Rule

A site needs at least 72 hours before being reused. 96 hours is better for daily protocols. 5–7 days for sites used with higher-volume injections like semaglutide or tirzepatide where the injection volume is larger.

If you feel hardness or tenderness at a site, give it an extra day regardless of your rotation schedule. Your protocol can absorb one day's adjustment. Injecting into inflamed tissue is not worth the minor scheduling inconvenience.

How many injection sites do I need?
At minimum 6 zones for once-daily subcutaneous protocols. For twice-daily dosing, use 8–10 zones so each site has at least 72 hours of recovery. More zones equals longer recovery time per site equals better absorption consistency over time.
What does lipohypertrophy feel like?
A hardened, rubbery lump under the skin. It forms gradually with repeated trauma to the same site. Early stages feel like a slight firmness. Advanced lipohypertrophy is clearly palpable as a raised knot. Once formed it does not fully reverse — it can shrink over months without injections, but the site is permanently less reliable for absorption.
Can I inject into the same site if the last injection didn't hurt?
No. Pain is not a reliable indicator of tissue health. Lipohypertrophy often forms without significant pain, especially in the early stages. Follow the rotation schedule regardless of how any particular injection feels.
Should I rotate injection depth (subcutaneous vs intramuscular)?
Most research peptides are injected subcutaneously. TRT testosterone is typically intramuscular. These use different anatomical targets and different needles, so they don't compete for the same sites. Rotate each injection type within its own set of appropriate zones.
